Parent Handbook

Our parent handbook gives details regarding all of our policies and procedures. We need the last page of this handbook signed and returned as part of registration.

Enrollment Form

This form is required for all families looking to enroll children. Please fill out this form completely and accurately. We need a schedule per child so that we can staff accordingly.

Health Appraisal Form

This form is required by the State if Michigan. We must have this form filled out, signed by a doctor and returned. This form will need to be updated once a year for children who are under 2 years old and every 2 years for children over 2 years old.
